Annual Girl Registration Fee Payment

The annual fee of $150 covers activities that may occur from September 1, 2011 to August 31, 2012. It is possible that your daughter’s unit will not meet during that full time period, but there are always activities happening in which your daughter would be able to participate. In addition to the annual registration fee, there may be additional costs associated with weekly meetings, special activities and outings with your daughter’s unit. Please speak with your daughter’s leader for information on these costs.

1. This form must be completed for each girl wishing to join and it must accompany the Membership (A.5) form.

2. If payment is not received with the form or is returned ‘NSF’, the registration will not be processed.

Families with more than two girls registering for Girl Guides of Canada-Guides du Canada programs are eligible for a sibling discount for the third and subsequent daughters. Full payment of $150 is due for each of the first two girls and for the third and subsequent daughters the fee is reduced to $75 per girl. If additional fee subsidy is required, a Fee Subsidy Application form must also be submitted.

CANCELLATION & WITHDRAWAL:

• A full refund will be issued if prior to October 1, 2011, Girl Guides of Canada, Ontario Council is unable to provide placement in a unit or a unit is closed due to low enrolment or for unforeseen circumstances. • A full refund less $15 administration fee will be issued if the Parent/Guardian

notifies the local community office

of the cancellation/withdrawal

prior to the start of the third scheduled unit meeting.

Notification from the Parent/Guardian may be made by phone, fax, e-mail or letter to the local community office. • No refunds will be considered

after the third scheduled unit meeting.

Refunds are not based on attendance at the Unit meeting. Payments are non transferable.
